Pony Realism 2.2 does tend to have a bias towards large breasts. I've found that without any mention of breast size in the Positive Prompt, putting "(sagging breasts:1.x)" in Neg can help quite a bit with reducing the size without making them too flat or androgynous, and I may combine that with Neg "(large breasts, medium breasts:1.x)" to get a shape and size that I personally prefer. (examples 1/4, 2/4). Normally I use typical values of x= 3-5. Also, you may find that Cyber Realistic does slightly smaller Boobs for any given prompt, (examples 3/4, 4/4), but otherwise gives quite similar results to Realism 2.2.
